Who knew cockroaches were so fond of ice cream? York County food safety inspections Jan. 10

The Pennsylvania Department of Agriculture oversees restaurant inspections in the state. Inspection reports are "snapshots" of the day and time the inspections took place. In many cases, violations are corrected on site prior to the inspector leaving.

The following establishments in York County that handle food were inspected during the week of Jan. 10-16, and were recorded as of Feb. 4.

In the spotlight this week:

SOUTH YORK DINER

2149 S. QUEEN ST., YORK Inspection due to complaint -- Out of complianceCracks or small holes were observed in various locations of the kitchen area.Observed one dead cockroach-like insect inside a bread storage drawer in the waitress station room.Observed one live cockroach-like insect crawling on wall surface behind a refrigerated bain-marie in the waitress station room.Observed two dead cockroach-like insects inside a drawer used to stored clean containers (creamer containers and stainless steel hot water pots for tea) used in coffee and tea service.Observed three dead cockroach-like insects behind the grills in the short order food preparation area.Observed dead cockroach-like insects too numerous to count underneath the hand-dipped ice cream freezer in the waitress station room.Unidentified debris found inside an open container of orange sherbet or ice cream inside the hand dipped ice freezer; ice cream or sherbet product appeared to have been thawed then refrozen (formation of ice) -- product disposed.Food debris, grease spillage and other debris found under and along the walls of virtually all equipment food service in the short order preparation area.Grease spillage/splatter and food debris found inside refrigerated cabinets under the grill, which had doors removed and refrigerated units no longer working.Observed gross accumulation of grease spillage under grills, on heat adjustment knobs on grills, on floors under and behind grills.Heavy accumulation of food splatter found inside a microwave oven in the short order preparation area.Food debris observed in door tracks on a refrigerator with sliding glass doors in the waitress station area and on the dessert case near the cashier station.Food debris (bread crumbs) and melted plastic bags were adhered to the pan surfaces on a tabletop steam table used to store/serve bread from in the waitress station area.Food debris found inside various drawers, shelving units and utility carts.Duct tape was being used to repair a broken handle on a meat tenderizer -- utensil disposed by owner.Carpet (rubber back area like rug) being used to line shelving in the warewashing area.Observed what appears to be a mineral deposit adhered to the surface of hot food wells on both the steam table in the short order preparation area and the tabletop units in the waitress station areas.Broken perforated plastic busing pans, which were cracked/broken and being held together with plastic tie strips, were being used as colanders to strain liquid from foods--these pans are no longer serviceable or cleanable.Improper hand washing -- food employee observed donning single-use gloves without a prior hand wash; observed an employee wash hands, turn off the water faucets, then dispense paper towels.Duties and responsibilities of the person in Charge -- food employees are not trained in food safety as it applies to their assigned duties as evidenced by improper hand washing and use of single use disposable gloves.

MILLENNIUM PIZZERIA

26 S. BALTIMORE ST., DILLSBURG Regular inspection - Out of complianceA can of butane and a bottle of simple green was stored above clean food equipment. Food employee observed in the kitchen area not wearing proper hair restraints, such as a net or hat. Squeeze and spray bottles of oil and water were not labeled with the common name of the food. Time in lieu of temperature was being used in the food facility to control pizza without written procedures or documentation to verify disposition of food. Prepackaged desserts near the front counter were not labeled properly with the name of product, ingredients, and sub-ingredients. There was an accumulation of ice on the fan unit in the walk-in cooler and in the upright freezer in the basement. Observed deeply scored cutting boards not resurfaced or discarded as required (red and green cutting boards). A toothbrush, tube of toothpaste and bottle of mouthwash found at the three-bay sink. The nozzle on the three-bay sink was not long enough to fill the two outer compartments of the sink -- facility has part ready to be installed. Light shields in the kitchen area were missing the end caps. The person in charge does not demonstrate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

PAKHA'S THAI HOUSE RESTAURANT

3 US RT 15 SOUTH, DILLSBURG Regular inspection - Out of compliance A can of butane was stored on the same shelf as food in the dry storage area. Various foods (chicken, beef, pork) were held at 51degF in the bain-marie, rather than 41degF or below, as required -- all food was discarded. Food was being stored in non-food grade bags in the walk-in freezer. The bain-marie, in the kitchen area, was not maintaining proper temperature. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 parts per million, and not 50-100 ppm, as required -- facility will use three-bay sink until dishwasher is fixed. An accumulation of grease was found in the hood filters above the fryer and grill area. Black residue found on the fan guard of the upright refrigerator in the kitchen area. Observed several small holes on the wall behind the grill area -- not a smooth and easily cleanable surface. The Person in Charge does demonstrate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

CHRISTIAN SCHOOL OF YORK

907 GREENBRIAR ROAD, YORK Regular Wood legs supporting the three-basin sink and wood shelving under the three-basin sink are not smooth, nonabsorbent, corrosion resistant, as required. An accumulation of static dust found on the light cover and fire suppression piping of the hood ventilation system. Three ant-like insects found in the faucet area of the three basin sink.

CONEWAGO ELEMENTARY SCHOOL

570 COPENHAFFER ROAD, YORK Regular Plastic spatulas were observed deformed and misshapen. Loose rubber door gaskets observed on the walk-in refrigerator cooler door.

JIM MACK'S ICE CREAM

5745 LINCOLN HIGHWAY, YORK Regular Antacids and cough drops were being stored on shelving next to single-use disposable food service containers -- corrected on the spot. Over-the-counter pain reliever and first aid items were being stored on the same shelf with food preparation equipment (stainless steel milkshake cups) -- corrected on the spot. Food employees with full facial hair (beard) observed involved in manufacturing bulk frozen dessert (hand dipped ice cream) not wearing proper hair restraints such as beard covers. Moist wiping cloths were balled up on the customer service counter and not soaking in sanitizing solution when not in use, as required. Duct tape was being used to repair ice cream freezer lid. A pink mold-like growth found on the deflector plate inside the bulk ice machine -- corrected on the spot. A incandescent light bulb inside the walk-in freezer was not shielded from breakage or constructed of shatterproof, as required.

RED LION BOWLING CENTER

90 MARTIN ST., RED LION Regular Bags of potatoes were being stored on the floor behind the customer service counter. Thermometers for ensuring proper temperatures of food were not available or readily accessible in a conspicuous location inside the reach-in refrigerator. Grill cleaning pad and grill cleaning tool where the pad attaches was being stored in the designated hand wash sink. Retail facility is approved for two basin warewashing, but must leave designated hand wash sink for hand washing only -- corrected on the spot.

ROUNDTOWN ELEMENTARY SCHOOL

570 CHURCH ROAD, YORK Regular Eight plastic spatulas, being used in daily food prep activities, were scored and discolored and not durable under this usage condition.

AMERICAN LEGION POST #543

23 E. BROADWAY, RED LION Regular A small soda syrup spill found underneath the bag-in-a-box rack in the basement. Clean food utensils (tongs) were stored hanging from natural gas lines in the kitchen -- not considered a clean storage location. There were missing floor tiles in the rented basement party room.

FESTIVE BOARD DELI

2031 SPRINGWOOD ROAD, YORK Regular Food employee involved in food preparation with full facial hair not wearing proper hair restraints such as beard covers. A butt end of deli meat was being displayed inside a deli display case unpackaged sitting directly on ceramic tiles. Cooked, prepackaged foods (meals to go) -- shrimp stir fry and meatloaf dinner - were not properly labeled with the name of product, ingredient statement including sub ingredients, major allergens, unit of sale, business name and address, as required. Unpackaged smoked, cured and nonsmoked deli meats were displayed for sale on cut pieces of ceramic floor tiles; these tiles generally are not food grade and are not approved as food contact services.

GENERAL NUTRITION

2899 WHITEFORD ROAD, YORK Regular Lean shakes and kale chips were displayed directly on the floor in the retail sale area, rather than 6-inches off of the floor as required.

GYRO-X-PRESS

55 MEMORY LANE, YORK RegularWorking containers of window and all-purpose cleaners were stored above and on the same shelf with single-use paper towels, coffee filters, coffee and souffle cups. Raw shell eggs were stored over ready-to-eat diced tomato mix in the walk-in cooler. An accumulation of grease found on hood ventilation system over the flat grill.

HAYSHIRE ELEMENTARY SCHOOL

2801 HAYSHIRE DRIVE, YORK Regular Ambient air and water temperature measuring device for ensuring proper food storage in the single door reach-in refrigerator in the kitchen area, not accurate to +/- 3degF. Handle on the drink cooler to the left side of the serve line was broken and should be replaced.

LOCUST GROVE ELEMENTARY SCHOOL

3620 E. PROSPECT ROAD, YORK Regular Reach-in refrigerator door handles are not cleaned at a frequency to preclude accumulation of dirt and soil. Clean and calibrated thermometers were stored in contact with scissors, writing markers, and various other items that were not cleanable and/or clean to sight and touch. Clean slicing, paring and various other types of knives were stored in cardboard sleeves; cardboard is not smooth, non-absorbent or cleanable as required for clean utensil contact surfaces. Employee jackets were hanging on and coming in contact with clean small wares and single-use plastic food storage bags on the shelving unit to the right of the walk-in cooler door. One of the eight fluorescent light bulbs in the dry storage area was not shielded or shatterproof.

MARCELLO'S PIZZA

421 CENTER ST., MOUNT WOLF RegularAn accumulation of static dust found on ceiling and light panels over the pizza preparation and baking areas. Time temperature controlled for safety (TCS) food were being stored in a refrigerated bain-marie unit with an ambient air temperature of 48degF; internal temperatures of TCS foods were: tuna salad, 53degF; sliced ham, 55degF; sliced turkey, 56degF; cheese 56degF; diced tomatoes, 55degF; 20 pounds of TCS foods disposed with an estimated value of $120; inspector's thermometer calibration verified in the presence of the owner before disposal. Shelving units and utility cart were lined with aluminum foil. Frozen condensate was leaking onto the floor and fan unit inside the walk-in freezer.

YMCA

500 GEORGE ST., HANOVER Regular No sanitizer test strips were located during inspection.

SUBWAY AT YORK SUNOCO

1000 S. GEORGE ST., YORK Regular| Broken cabinet doors under the beverage dispensing counter on the retail floor. The auto fill/mixing sanitizer dispensing at the three basin sinks was dispensing 0 parts per million (ppm) quaternary ammonia, when required to dispense at 200 ppm -- IMMEDIATE CORRECTIVE ACTION IS REQUIRED. Employee was able to manually add additional sanitizer by pouring from the bottle into the sink, but the auto mixing and dispensing system was not working -- corrected. Paper towel dispenser empty at the hand wash sink in the customer restroom -- corrected on the spot.

IMPERIAL GOURMET

227 PAULINE DRIVE, YORK Follow-up Food facility has an employee that has taken food safety training program; however, the food safety program was not an ANSI/CFP accredited Certified Food Manager program. A wet wiping cloth in kitchen area was not being stored in sanitizer solution. Floor in the kitchen area under refrigerator, bain-marie and dishwasher is made of tile and was cracked/broken, with pieces missing and not a smooth, easily cleanable surface. The floor/wall juncture to the right of the rear exit door was not coved and closed to 1/32 inch.

NATURAL CHOICES

2241 INDUSTRIAL HWY, YORK Regular Ginger cookies were stored directly on the floor in in the rear storage area, rather than 6-inches off of the floor as required.

ORE VALLEY ELEMENTARY SCHOOL

2620 SPRINGWOOD ROAD, YORK Regular Interior casing of reach-in refrigerator had separated from the door frame exposing insulation between interior and exterior door panels; work order placed during this inspection. An accumulation of frozen condensate found on the left side of the fan unit located in the walk-in refrigerator; manager stated that a work order has been placed.

PERKINS RESTAURANT

300 EISENHOWER DRIVE, HANOVER Regular Facility does not have procedures for employees to follow when responding to an event involving vomitus or fecal matter discharge onto surfaces within the facility -- provided handouts. Food employee observed in short order grill area wearing bracelet/watch/ring on hands or arms - corrected on the spot. Sticker residue found on plastic food storage containers - corrected on the spot. Hood area over the grill had some grease buildup.

ROYAL FARMS

101 CARLISLE ST., HANOVER Regular Four quarts of milk were dated Jan. 11, 2016, on shelf for sale to customers; valued at $1.99 each -- corrected on the spot (inspection date was Jan. 13, 2016). Rusting shelving found in the reach-in cooler; new shelving has been ordered. Dried chicken juices found on the chicken transport cart - corrected on the spot. Paper towel dispenser was not working properly in the food service area - corrected on the spot. Static dust found on ceiling vents in food prep areas.

TOURIST INN

671 W. MARKET ST., HELLAM Regular Water was pooling under the soft drink carbonator located in the basement. Water was leaking from underneath the high temperature warewashing machine. An accumulation of static dust found on the fan guard and fan blades of an electric floor model fan being used to cool or remove heat from the compressor on the walk-in freezer; fan is located very close to single-use disposable clam shell food storage containers. Wall-to-wall carpeting, inside the ladies restroom, is not considered an easily cleanable surface.

YORK CO SCHOOL OF TECH

2179 S. QUEEN ST., YORK Regular Food employees were eating in food preparation area as evidenced by partially consumed food on a shelf beside clean food service pans in the kitchen area. Water temperature at the hand wash sink, located in the warewash room, was 127degF, which is too hot for employees to comfortably follow proper handwashing procedures; employees will use sink located through the open doorway in the next room until water temperature is properly adjusted. An accumulation of static dust found on the floor fan guard located in the warewashing room.

YORK TWP ELEMENTARY SCHOOL

2500 S. QUEEN ST., YORK Regular Stainless steel pans, stored as clean, had food tape residue and were not clean to sight and touch.

ARBY'S

409 LOUCKS ROAD, YORK Regular Heavy accumulation of frozen condensate on interior ceiling of upright freezer beside the fryer station

.BRUSTER'S

1 TRISTAN DRIVE, DILLSBURG Regular Ice cream scoops observed stored in a container of standing water which was not maintained at 135degF. Tape used on chest freezers is not smooth easily cleanable and is accumulating residue. Observed black residue on the pipe near the ceiling inside the ice machine.

CITIZEN'S HOSE CO #1

109 S. BALTIMORE ST., DILLSBURG Regular Food facility does not have available chlorine sanitizer test strips or test kit to determine appropriate sanitizer concentration.

FRIENDLY'S

423 LOUCKS ROAD, YORK RegularMarinara and cheese buffalo sauces that require temperature control for safety were held at 106degF, in the right steamer well, rather than 135degF or above, as required. Loose rubber door gaskets observed on both lower bain-marie cooling unit doors; manager stated that replacement gaskets have been ordered. Accumulation of frozen condensate on and around fan unit inside the walk-in freezer. Walk-in cooler door handle was broken and wrapped in nonfood grade tape; this handle is no longer a smooth, nonabsorbent, easily cleanable surface, as required.

LOGAN'S ROADHOUSE RESTAURANT

2180 YORK CROSSING DRIVE, YORK Follow-up Server hand sink was being used for other tasks than handwashing; it was stained from tea and coffee. Floor in the bar area is deteriorating, exposing gravel from the concrete; manager provided a contractor bid for the repair and stated the floor repair will be completed.

PARADISE ELEMENTARY SCHOOL

6923 LINCOLN W. HWY, THOMASVILLE Regular Thermometers were being stored in container of sanitizer - corrected on the spot.

RAMBLER INN

6600 LINCOLN W. HWY, THOMASVILLE Regular Food facility is using/offering for sale foods prepared in an unapproved private home (chocolate-type candy). Exhaust hood filters are made of the mesh type screen and cannot be properly cleaned. Soda gun and holster had food residue and were not clean to sight and touch - corrected on the spot. Dirty standing water found in basin under the container of ice behind the bar.

BRUSHTOWN ATHLETIC ASSOC

5603 HANOVER ROAD, HANOVER Regular Food facility did not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration.

GOODE SCHOOL

251 BROAD ST., YORK Regular Storing clean food equipment while wet, and not allowing time for draining and/or air-drying. Toilet room not provided with a covered waste receptacle for sanitary napkins.
